Title: NEWB SIM: A non-simulation-trained and non-subject-matter-expert-led simulation to prepare event medical teams

Introduction Educator training in simulation facilitation/debriefing can be costly and time-consuming. In event medicine, in-situ simulation is often limited by access to experienced facilitators and subject experts.
Objective This program evaluation explores whether educators without formal simulation training or topic expertise can effectively run simulations using a facilitation/debriefing blueprint. The blueprint provides cues for case advancement and debriefing questions.
Methods “New Educator With Blueprints Simulation” (NEWB SIM), was piloted to support first-time simulation educators. Educators (e.g., first aiders/responders, medical students, nurses) received a NEWB SIM Facilitator/Debrief Guide. A complex cocaine toxicity case with chest pain, hyperthermia, muscle rigidity, and cardiac arrest was facilitated for interdisciplinary teams of 4-6.
Results 19 of 23 participants (83%) completed a post-program evaluation. Mean scores were calculated for various indices: case facilitation quality (89%), support of new facilitators using scripted cases (98%), overall debriefing quality (88%), teamwork debriefing quality (90%), medical debriefing quality (92%), stress-free debrief environment (82%), support of new facilitators using scripted debriefs (91%), and support for using non-trained, non-expert facilitators (87%). All six educators (100%) completed a post-program evaluation. Mean scores were calculated for various indices. Baseline self-rated case facilitation competency was 35%, while educators thought participants would rate their facilitation competency at 58%. Baseline self-rated debriefing competency was 55%, while educators thought participants would rate their debriefing competency at 67%. The NEWB Facilitation/Debrief Guide ease-of-use scored 75%. Educators’ stress-free level was rated 67% during case facilitation and 73% during debriefing. Support for using non-trained, non-expert facilitators was rated 68%.
Conclusions Educators felt less competent in facilitation/debriefing skills than participants perceived, as evidenced by participants' high ratings of educational quality and support for scripted cases/debriefs. The NEWB SIM Facilitator/Debrief Guide effectively assists educators, making debriefs a moderately stress-free experience. All endorse using non-trained, non-expert educators, which is valuable in low-resource settings with limited access to trained/expert personnel.
